2. Create a Behind-the-Scenes Google Form
This Google Form is only for storing responses and is NOT visible to respondents.
STEP 1
Create a blank Google Form at docs.google.com/forms
STEP 2
In the Google Form, create a Question for each of your Questions in order.
Each Question should have the Google Form type “Short Text”.
STEP 3
Publish the Google Form
Note: The Google Form is only for storing responses. It is not visible to respondents.

- Link the Google Form to a Spreadsheet
- Turn on Email Notifications

- Collect email addresses via Settings
- Require Sign-In
- Mark Google Questions as Required
3. Add Links from the Google Form to this Form
STEP 1
Open Google Form's Responder Link (NOT the Preview Link).
Wait for the google form page to completely finish loading, as the relevant HTML doesn't load immediately.
STEP 2
Extract the Google Form’s Action URLs and Input Names.
Extraction can be done manually, or with the below tool.
STEP 3
Add Google Form’s Action URL and Input Names to your Form
